Specifically, Van Voorhis, who is the manager and operator of Ultimate Cleaning Limited, a commercial cleaning business located in Appleton, Wisconsin, admitted that, during the period from January 2012 through June 2016, he failed to pay over to the Internal Revenue Service federal more than $200,000 in payroll taxes that had been withheld from the wages of employees of Ultimate Cleaning.

Van Voorhis also failed to file quarterly payroll tax returns during this period.

Van Voorhis, age 52 faces up to 5 years in prison and a fine of up to $250,000, as well as having to pay the unpaid taxes. Van Voorhis is scheduled to be sentenced on November 2, 2018.
